Walk in assessments Monday through Thursday 9 am to 1 pm, or set up appointments online at www.sageprairie.org. Day Intensive Outpatient treatment with or without patient housing for Men and Women. Monday through Thursday 9 am to 3:40 pm, with individual counseling outside of these hours. Care coordination for medications-including Suboxone, Sublocade, Brixadi and Vivitrol with the Sage Clinic or other area clinics. Facility to facility transfers within a 50 mile radius. Individualized program, usually (approximately) 90 days or more in length, followed by stepdown to less intensive programming. Sage Prairie housing exclusive to Sage Prairie or patients partner or other partner in the community (depending on individual needs of client).
