Walk in assessments Monday through Thursday 9 am to 1 pm, or set up appointments online at www.sageprairie.org. Day Intensive Outpatient treatment with or without patient housing for Men and Women. Monday through Thursday 9 am to 3:40 pm, with individual counseling outside of these hours. Care coordination for medications-including suboxone, sublocade, and vivitrol with the Sage Clinic or other area clinics. Facility to facility transfers within a 50 mile radius. Individualized program, usually (approximately) 90 days or more in length, followed by stepdown to less intensive programming. Patient housing can extend beyond programming for $550/month, depending if the housing is SP exclusive partner or other partner in the community (depending on individual needs of client). Peer support onsite or referred to a partner in Dakota County (if needed).
