Community Foundations is an Intensive Residential Treatment Services (IRTS) program located in Maplewood, MN. Staff provide recovery-oriented, evidence-based treatment and rehabilitative services for sixteen people struggling with acute symptoms of mental illness and/or co-occurring substance use disorders. Each person has their own bedroom and bathroom; this allows for privacy, dignity, and supports the person in providing a peaceful recovery environment. Services include:

• Individualized assessment
• Illness management
• Integrated services for mental illness and substance use
• Family education
• Crisis assistance
• Development of health care directives and crisis prevention plans
• Nursing services
• Inter-agency case coordination
• Client transition and discharge planning

Specific services and duration of services is based on clinical need. The program has a maximum capacity for sixteen individuals.
