Referrals can be faxed to: 763-427-6876 or emailed to referrals@riverplacecenter.com. To ensure no delays in processing your referral please provide the level of care you are requesting, client’s insurance/funding information, medications they will be coming to the program with, a way to reach the client, and contact information for yourself in the event we have questions.
