The Recovery Hub is the outpatient program for the Transformation House continuum of care. We are a behavioral health care provider specializing in substance use disorders and mental health treatment services. We are located in Anoka, MN, and offer both in-person and telehealth programming.

Our multidisciplinary team takes a holistic approach to each individual’s diagnosis, cultural and socio-economic background, to identify needed changes, set goals for the future, developing client-centered treatment plans
