Skills that have been lost or diminished due to symptoms of mental illness can be acquired, practiced and enhanced through training and education. Our quali\ufb01ed ARMHS practitioners provide one-on-one support both in the home and in the community. (Funded by MA) ARMHS practitioners instruct, assist and support people with many skills and goals, such as:<\/p>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<div class=\"et_pb_row et_pb_row_3\">\n<div class=\"et_pb_column et_pb_column_1_2 et_pb_column_4 et_pb_css_mix_blend_mode_passthrough\">\n<div class=\"et_pb_module et_pb_text et_pb_text_4 et_pb_text_align_left et_pb_bg_layout_light\">\n<div class=\"et_pb_text_inner\">\n<ul>\n<li>Mental illness symptom management;<\/li>\n<li>Healthy lifestyle decisions;<\/li>\n<li>Interpersonal communication;<\/li>\n<li>Household management;<\/li>\n<li>Community resource utilization and integration;<\/li>\n<li>Crisis assistance;<\/li>\n<li>Relapse prevention;<\/li>\n<li>Cooking and nutrition;<\/li>\n<li>Transportation access;<\/li>\n<li>Medication education and monitoring;<\/li>\n<li>Employment-related skills; and<\/li>\n<li>Transition to community living services<\/li>\n<\/ul>\n<\/div>\n<\/div>\n<\/div>\n<div class=\"et_pb_column et_pb_column_1_2 et_pb_column_5 et_pb_css_mix_blend_mode_passthrough et-last-child\">\n<div class=\"et_pb_module et_pb_text et_pb_text_5 et_pb_text_align_center et_pb_bg_layout_dark\"><\/div>\n<\/div>\n<\/div>\n<div class=\"et_pb_row et_pb_row_4\">\n<div class=\"et_pb_column et_pb_column_4_4 et_pb_column_6 et_pb_css_mix_blend_mode_passthrough et-last-child\">\n<div class=\"et_pb_module et_pb_text et_pb_text_6 et_pb_text_align_left et_pb_bg_layout_light\">\n<div class=\"et_pb_text_inner\">\n<p>Individuals interested in receiving ARMHS must meet the following state mandated criteria:<\/p>\n<ul>\n<li>Be age 18 or older;<\/li>\n<li>Live in Hennepin, Ramsey or Dakota Counties;<\/li>\n<li>Be on medical assistance or PMAP;<\/li>\n<li>Be diagnosed with a serious mental illness (SMI) such as Major Depressive Disorder, Bipolar Disorder, Schizophrenia, Schizoaffective Disorder, or Borderline Personality Disorder; and<\/li>\n<li>Have a functional impairment in 3 or more life domains due to the symptoms of the diagnosed SMI.<\/li>\n<\/ul>\n<p>To apply for these services, please complete the <a class=\"et_pb_button et_pb_button_0 et_pb_bg_layout_light\" href=\"https:\/\/www.reachforresources.org\/wp-content\/uploads\/2022\/01\/ARMHS-Referral-Form-Jan-2022.pdf\" target=\"_blank\" rel=\"noopener\">ARMHS REFERRAL FORM<\/a>\u00a0 \u00a0or go to <a href=\"https:\/\/www.reachforresources.org\/services\/mental-health-services\/armhs\/\">Adult Rehabilitative Mental Health Services (ARMHS) \u200b &#8211; Reach For Resources <\/a>and submit it via email to <a href=\"mailto:armhs@reachforresources.org\">armhs@reachforresources.org<\/a>\u00a0or via fax at 952-229-4468.
