Walk in assessments Monday through Thursday 9am to 1 pm. Day and Evening Intensive Outpatient treatment. Set up ontake/assessment online at www.sageprairie.org. Group counseling schedule is Monday and Wednesday 12-3 pm/5:30-8:30 pm and Thursday 12-2 pm/5:30-7:30 pm, with individual counseling outside of these hours. Care coordination for medications-including suboxone with the Sage Prairie Clinic and other clinics in the area. Individualized program, usually approximately 80-120 hours, followed by stepdown to less intensive programming. Peer support onsite or referred to a partner in Dakota County if needed.
