High Fidelity Wraparound is a team-based care coordination process that honors and builds on family culture. The family, Wraparound Team, and community providers partner to develop an individualized plan that supports access to resources and informal supports and builds confidence in their ability to problem-solve in the future.<\/p>\n<ul>\n<li>Primary address must be in Hennepin County.<\/li>\n<li>A Diagnostic Assessment must be completed within the last 180 days to determine eligibility.
