Volunteers of America of MN – High Fidelity Wraparound

Wraparound supports youth and their families with complex mental health needs by focusing on their strengths and support system. High Fidelity Wraparound is a team-based care coordination process that honors and builds on family culture. The family, Wraparound Team, and community providers partner to develop an individualized plan that supports access to resources and informal supports and builds confidence in their ability to problem-solve in the future.

• Primary address must be in Hennepin County.
• A Diagnostic Assessment must be completed within the last 180 days to determine eligibility. VOA can support you in completing an updated Diagnostic Assessment, if needed.
• Virtual and in-person options available.
• Serve ages 6-17
• Serve all genders
• This program is free to participants and is financially supported by Medical Assistance, PMAP providers and funding from Hennepin County for uninsured or privately insured youth.
